Our dedicated team of medical professionals offer you support and guidance as well as skilled medical care. We will work with you and your physician to develop a home care plan that includes education and training for you and your family so you can continue your recovery in between our visits. Our team includes:
- Primary care physician to provide guidance, leadership, and quality assurance of the care you are receiving.
- Registered nurses to provide one-on-one care in your home and monitor your progress to make sure you are getting the best-individualized care possible.
- Physical therapists to assess and treat movement problems and help you regain mobility through at-home exercises.
- Speech therapists to work with patients who may have trouble speaking following a stroke or brain injury.
- Occupational therapists to work on your independence and fine motor skills with daily living activities such as being able to feed and dress yourself.
- Medical social workers to make sure you have the education and resources to get the medical, psychological, and social assistance you need.
- Home health aides to work alongside your nurse and help with basic healthcare needs such as changing bandages, checking your temperature and pulse, and helping you with artificial limbs or walking aids.
