Who is eligible for the program?
Eligibility is dependent on the care team and includes factors such as a patient's condition, diagnosis, and severity of illness. You must be age 18 or older to be a part of the program.
What care would I receive in the program?
If you or a loved one is identified as a candidate, you will receive hospital-level care from a Health First care team.
This includes:
- Daily visits in the home by a registered nurse
- Visits in the home by a Physician Assistant or Nurse Practitioner
- In-person or virtual visits with a physician
- Remote patient monitoring
- Delivery, set up, and instruction on all medical equipment, supplies, and medication needed during care.
- A dedicated coordinator, who will guide your care.
- Our Case Management team will work with you to provide any additional resources from our support teams, such as Physical Therapy, Social Services, and Dietitians.
- Our Transitional Care team will ensure you have a smooth transition through the discharge process, including helping to schedule follow-up appointments.
- Other services offered include lab draws, radiology support, pharmacy support, and pastoral care.
Will a care team visit me at home?
You will have daily in-person visits from one of our skilled nurses, in-home visits from a physician assistant, and medical providers. You will be able to reach your care team at any time with access to a telehealth video platform, for any additional needs.
What is Remote Patient Monitoring?
Remote Patient Monitoring connects you to your provider and care team, using the latest technology to observe health data while you are receiving care. It gives your care team the power to know what is occurring with your care in real time.
How long can I receive care at home?
Care will be provided at home for as long as treatment is needed. Once you are ready, you will be discharged from the program.
Does it cost more to be a part of this program versus receiving treatment at the hospital?
There is no additional cost compared to receiving treatment at a traditional hospital.
Is my insurance accepted?
Depending on the type of insurance you have; you may be eligible for this program. The coordinator will evaluate whether you are eligible for this program while you are being evaluated in the ER.
Is this program different from Home Care?
This program is an inpatient program, with the only difference being the location of treatment and care. Home Care is a short-term outpatient program available for anyone who has a medical condition requiring skilled care that cannot be provided solely by family members or friends.
What other At-Home services are offered at Health First?
We offer outpatient, at-home services including Home Care, Hospice, and Private Duty. These services may be provided, if needed, after you are discharged.
