The Health First Community Health Needs Assessments (CHNA) were conducted in compliance with federal tax-exemption requirements and the Affordable Care Act. Based on the CHNA's findings, the Health First Community Benefit Steering Committee, consisting of Health First leadership and the Board of Trustees, identified two major disparities in Brevard County:
- Adults without access to healthcare services.
- Overweight/obesity and poor nutrition/lack of physical activity, particularly among children and the underserved population.
Our dashboard measures the monthly progress of the implementation strategies created to address these disparities.
