To obtain access to Protected Health Information (PHI) Systems through Health First, a provider practice or business must complete a Universal Access Agreement (UAA). If you have questions pertaining to a previously executed agreement, or need further assistance, please e-mail UAA@hf.org.
Step 1: Execute Universal Access Agreement (UAA)
A provider practice or business must complete the UAA agreement. To begin, identify an "Authorized Representative" for the practice or business, (e.g. office manager or practice owner). The "Authorized Representative" is responsible for managing the agreement terms of the UAA to include requesting access for new users and notify Health First to terminate a user's access.
To initiate the UAA, select one business relationship to Health First:
- Health First Health Plans: HFHP credentialed provider
- Health First Medical Staff: Medical staff credentialed provider
- Community: Provider or business not credentialed with Health First
Step 2: Complete User Attestation
Each employee within the provider's practice or business requesting access will need to complete a User
Attestation. The "Authorized Representative" identified within the UAA must initiate this process.
