Clyde Blaylock is a board certified nurse practitioner with several years of experience in the medical field, the majority spent as a Surgical Assistant in the operating Room and in Critical Care, Pulmonary and Medical-Surgical Nursing.

Clyde earned his Masters in Science in Nursing-Family Nurse Practitioner from the University of Colorado at Colorado Springs. His Bachelor of Science in Nursing, B.S.N, was earned from the University of Central Florida in Orlando, FL. He also holds an Associate of Arts degree from Brevard Community College in Cocoa, FL and is a graduate of the School of Healthcare Sciences of Surgical Technology, United States Air Force, Sheppard Air Force Base in Texas.

Before joining Health First Medical Group, Clyde worked with Independent Lung Associates Pulmonary, Critical Care, and Sleep Medicine in Melbourne, FL. He has also served as Surgical Services Manager/House Supervisor with Kindred Hospital in Melbourne and as a Registered Nurse/Orlando Clinical Supervisor with Bassin Center for Facial Plastic Surgery in Viera and Orlando, FL.

Clyde also spent time as an Orthopedic Sales Representative for Orthopedic Fixation Systems in Palm City and as Cardiac Cath Lab Manager/Operating Room Educator/Coordinator/Ambulatory Care Manager for Wuesthoff Medical Center Melbourne. Additional positions include working as Director of Nursing/Operating Room Manager/Infection Control Coordinator for HealthSouth Surgery Center, Registered Nurse for Wuesthoff Medical Center - Rockledge, Certified Surgical Technologist for Melbourne Same Day Surgery Center, Certified Surgical Technologist for Cardiac Surgery at Wuesthoff - Rockledge, Surgical Assistant with Dr. Vitale-Lewis, Certified Surgical Technologist with Health First's Holmes Regional Medical Center and Surgical Technologist with the United States Air Force at Kessler Air Force Base in Mississippi.

Clyde is from the Melbourne area, graduating high school from Melbourne Senior High School. He is an American Academy Nurse Credentialing Board Certified Nurse Practitioner and has earned both Basic Life Support and Advanced Cardiac Life Support Certifications from the American Heart Association.

Policies

New patients should arrive at least 15 minutes before their scheduled appointments to complete all paperwork. They should bring their insurance cards, a government-issued photo ID, and lists of their medicines and any prior operations. Co-pays and any outstanding balances are due at the time of each visit. Generally, operations or procedures won't take place on the first visit.
