Dr. Harry Diaz is Board Certified by the American Board of Family Medicine. He earned his Doctor of Medicine from Universidad Central Del Caribe, Cayey/Bayamon Puerto Rico. He earned his undergraduate degree at Universidad Interamericana de Puerto Rico, Metropolitan Campus, Rio Piedras, Puerto Rico. His residency was completed in Family Medicine at Hospital San Pablo, where he was also elected Chief Resident by his peers and faculty.

Most recently, Dr. Diaz was serving as Attending Physician with Urgent Care of Omaha and with One World Community Health Centers in Omaha, NE. He has nearly 20 years of experience working as a physician in Omaha, NE. He also has extensive experience serving as an attending physician in Puerto Rico.

Dr. Diaz is a Civil Surgeon for Immigration and Naturalization Services, USA Department of Homeland Security. While in Nebraska, he was a Volunteer Assistant Professor in the Family Medicine Department at the University of Nebraska School of Medicine. In Puerto Rico, he served as Assistant Professor at the Puerto Rico School of Medicine.

Office Lcoation


New patients should arrive at least 15 minutes before their scheduled appointments to complete all paperwork. They should bring their insurance cards, a government-issued photo ID, and lists of their medicines and any prior operations. Co-pays and any outstanding balances are due at the time of each visit. Generally, operations or procedures won't take place on the first visit.
