Dr. Chirag Faldu is a board-certified Nephrologist. He received his medical degree from the Saba University School of Medicine, after which he completed an internship in Internal Medicine and a fellowship in Nephrology at the University of Florida in Gainesville, Florida. Dr. Faldu graduated with Summa Cum Laude in Computer Science from North Carolina State University in Raleigh, North Carolina and was nominated for fellow of the year by the medicine house staff in 2011.

Dr. Faldu is a member of the American Society of Nephrology and the Florida Society of Nephrology.

Policies

New patients should arrive at least 15 minutes before their scheduled appointments to complete all paperwork. They should bring their insurance cards, a government-issued photo ID, and lists of their medicines and any prior operations. Co-pays and any outstanding balances are due at the time of each visit. Generally, operations or procedures won't take place on the first visit.
