We want you to feel empowered to plan ahead and make choices that are best for you. That's why we're focused on making your healthcare costs transparent and clear. 

Here's what you will need to get a price estimate:

  • Complete contact information, including name, address, email, and a valid phone number.
  • Gather your health insurance information including payer name, policy number, and group number, if applicable.
  • The CPT code for the service. Your physician's office can provide the code for you. This is necessary to ensure the estimate given is for the correct service.

For the latest information regarding Florida's healthcare costs, and to help you make more informed healthcare decisions, visit the Agency for Health Care Administration's Florida Health Price Finder. The Agency for Health Care Administration places an emphasis on healthcare transparency for Florida's consumers. The service bundle information is a non-personalized estimate of costs that may be incurred for anticipated services. Actual costs will be based on services actually provided. 

Good Faith Estimate

You have the right to a Good Faith Estimate. This means that if you do not have insurance, or if you are not using insurance, you can receive an estimate of the total expected cost of non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Financial assistance.

We understand that sometimes, financial difficulties happen and can affect your ability to pay for care. We want to help deliver the healthcare you and your family need, regardless of your situation. 

We offer financial assistance on a case-by-case basis. This can help cover the cost of healthcare expenses for eligible individuals who are uninsured, underinsured, ineligible for a government program, or otherwise unable to pay, due to limited financial resources.
