Post-COVID-19 conditions.
Some people who have been infected with the virus that causes COVID-19 can experience long-term effects from their infection, known as post-COVID conditions (PCC) or long COVID. It’s important to note that post-COVID conditions go by many names, including: long COVID, long-haul COVID, post-acute COVID-19, post-acute sequelae of SARS CoV-2 infection (PASC), long-term effects of COVID, and chronic COVID. For simplicity, we refer to the condition as long COVID.
Long COVID symptoms may include the following:
- Difficulty concentrating
- Shortness of breath
- Fatigue
These conditions need to occur for four weeks or longer after a confirmed COVID-19 diagnosis to be considered long COVID. For the latest information about long COVID, please visit the CDC website:
Program enrollment.
Regaining a sense of wellness after long COVID will be a personal journey. Our Post-COVID-19 Care program supports you with a customized care plan that will help you set and meet individual wellness goals.
To be eligible for the program, you must meet the following conditions:
- 18 years of age or older
- History of a COVID-19 diagnosis with symptoms occurring for four weeks or longer after the confirmed diagnosis

About the virtual intake session.
The intake session will consist of questions and assessments designed to better understand your COVID-19 symptoms and experience. Please allow up to 30 minutes for your intake session.
Once you are enrolled in the program, you will meet with our dedicated provider who will work with you to develop a customized care plan. You will receive a call to discuss any testing requirements and further coordination of care with the program, your PCP, and/or other specialists.

It's important to remember that you are not alone. It's estimated that millions of people around the world are living with long COVID. Estimates of prevalence vary with between 10% to 40% of people who contract COVID-19 likely to develop long COVID.
