Let MOM be your guide. Health First's Maternity On to Motherhood (MOM) Program is dedicated to supporting maternal and infant health across Brevard County through personalized, evidence-based, and compassionate care coordination.

The MOM Program connects you with care, resources, and support at every step. Services are provided at no cost and are open to all expecting mothers and families.

Program services include:

  • Coordinating care and referrals for appointments, services, and follow-up visits.
  • Creating a personalized birth plan that reflects your preferences and supports a positive birthing experience.
  • Hospital preregistration and admission preparation to help ensure a smooth arrival when it’s time to deliver.
  • Arranging guided tours of the Birth Suites at Holmes Regional Medical Center and Viera Hospital.
  • Assistance with selecting a pediatrician and understanding your baby’s first appointments.
  • Personalized guidance during pregnancy, birth, and after you return home.
  • Access to breastfeeding support, including lactation consultations, breastfeeding classes, and support groups.
  • Childbirth, baby care, and postpartum education to help you feel confident and prepared.
  • Connection to community resources and programs, including wellness services, parenting support, and family assistance programs.
  • Car seat education and installation support, so you feel safe and ready for your baby’s first ride home.

Yesenia has dedicated her nursing career to caring for mothers, babies, and families across obstetrics (OB), pediatrics, and neonatal care.

Fluent in English and Spanish, Yesenia is committed to bridging language and cultural gaps so every mother feels heard and understood. She takes the time to learn what matters most to you and helps ensure your care reflects your needs and preferences. Her goal is to help you feel informed and confident throughout pregnancy, birth, and your transition into motherhood.

A new baby is a major life change. Natural mood swings and high emotions sometimes lead to a more serious mental health issue. It truly does take a village to raise a child. In the days and weeks after giving birth, it is crucial to get the support you need. Rely on others to help you recover and transition into your role as a mother of one, two, or more.
