Provider Partnership Program
The Provider Partnership Program is an income guarantee program designed to support eligible physicians who establish a full-time practice in an approved specialty and Health First service area in Brevard County, Florida.
The program provides financial support during the early stages of building a practice, helping physicians establish themselves while expanding access to needed healthcare services in our community.
How the Income Guarantee Works
During the guarantee period, Health First provides financial support when eligible practice collections do not reach the agreed-upon monthly income target, plus approved covered expenses.
This support helps bridge the gap as a physician builds a patient base and grows practice revenue in Brevard County.
Community Need Guides Program Eligibility
Provider Partnership Program opportunities are based on documented healthcare needs within Brevard County.
Using findings from the 2026 Community Health Needs Assessment, Health First identifies specialties and geographic areas where additional physicians may be needed. The assessment projects physician supply and demand through 2029 and considers factors including population growth, an aging population and anticipated physician retirements.
Program eligibility and individual opportunities are based on identified community need and applicable program requirements.

Program Requirements
Eligible physicians must:
- Establish and maintain a full-time practice in the approved specialty and service area.
- Maintain required Florida licensure, registrations and appropriate medical staff privileges.
- Participate in Medicare and Florida Medicaid and complete applicable payor credentialing.
- Provide required call coverage and comply with applicable laws, standards and medical staff requirements.
- Submit accurate monthly receipts and expense documentation during the guarantee period.
- Remain in the service area and meet all program obligations for the required contract term to receive loan forgiveness.
How Financial Support May Work
Here is an illustrative example of how monthly support could be calculated:
Annual income guarantee: $360,000
Monthly income target: $30,000
Eligible covered expenses: + $5,000
Physician receipts: – $20,000
Illustrative monthly support: $15,000
